patients and develop individualized therapy plans Provide in-home
physical therapy treatments Improve mobility, strength, balance,
and functional independence Educate patients and caregivers on
exercises and safety techniques Monitor progress and adjust
treatment plans as needed Complete accurate and timely
documentation What We’re Looking For: Active Texas Physical
Therapist (PT) license (required) Home health experience preferred
Strong clinical judgment and patient care skills Ability to work
independently in a home setting Excellent communication and
documentation skills Reliable transportation & valid driver’s
license Kinnser/WellSky EMR experience (preferred) Pay & Perks: Up
to $75 per visit Flexible, self-managed schedule Independence in
