As part of our in-house therapy model, you’ll collaborate closely with nursing, leadership, and other rehab team members to make a direct, positive impact on resident lives—every day. Position Overview As our PRN Physical Therapist, you will play a key role in evaluating residents, developing personalized treatment plans, and delivering evidence-based interventions to support mobility, independence, and overall well-being. You’ll work alongside a dedicated Interdisciplinary Team (IDT) and contribute to a therapeutic environment built on compassion, communication, and clinical integrity. Key Responsibilities Perform comprehensive evaluations of residents’ physical conditions and functional abilities. Develop and implement customized treatment plans using evidence-based practices. Provide therapeutic interventions to improve mobility, balance, strength, and functional independence. Monitor progress and adjust care plans in collaboration with the IDT. Educate residents, caregivers, and families on mobility techniques and home exercises. Ensure timely and accurate documentation in line with facility and regulatory standards. Participate in team meetings and support an integrated care approach. Adhere to all safety, compliance, and regulatory requirements. Qualifications Active Texas Physical Therapist (PT) license. Good standing with all relevant licensing boards/agencies. SNF or long-term-care experience preferred (not required). Familiarity with Net Health documentation system is a plus. Strong communication and interpersonal skills. Ability to work effectively with diverse adult and geriatric populations. Why Join Conroe Health Care Center?
