Occupational Therapist provides expert therapeutic services to
patients in their homes, focusing on enhancing their ability to
perform daily activities and improving their overall quality of
life. In this role you will assess, plan, and implement
individualized therapy programs in collaboration with patients,
families, and other healthcare professionals to address physical,
cognitive, and emotional challenges. Your role is essential in
helping patients regain independence and achieve their personal
goals in a home environment. Schedule: PRN, Monday - Friday, 8:30
AM - 5:00 PM Primary Responsibilities and Duties: Conduct
comprehensive assessments of patients' physical, cognitive, and
emotional needs to develop personalized therapy plans. Design and
implement individualized therapy programs to address impairments,
functional limitations, and performance deficits. Educate patients
and their families on techniques and strategies to improve daily
living skills and adapt their environment to support functional
independence. Utilize therapeutic interventions, including
exercises, adaptive equipment, and modifications to daily
activities, to enhance patients’ abilities and quality of life.
Monitor and document patient progress, response to therapy, and any
changes in condition, providing detailed reports to the supervising
physician and other healthcare team members. Collaborate with
physicians, nurses, social workers, and other healthcare
professionals to ensure a holistic and coordinated approach to
patient care. Provide guidance on the use of assistive devices and
adaptive technologies to support patients’ functional abilities.
Maintain accurate and up-to-date patient records and documentation
in compliance with regulatory and organizational standards. Offer
support and education to caregivers and family members to improve
patient care and facilitate the therapy process. Participate in
ongoing professional development and stay current with advancements
in occupational therapy practices and techniques. Perform all other
duties assigned. Requirements: Job Qualifications: Current state
licensure as an Occupational Therapist. 1 year of clinical
experience in a home health facility or similar healthcare setting
is preferred. Ability to build rapport with residents through a
compassionate and caring demeanor. Excellent interpersonal and
communication skills, with the ability to collaborate effectively
with residents, families, and team members. Commitment to providing
high-quality, resident-centered care with empathy and compassion.
Strong clinical skills in assessment, treatment planning and
intervention implementation. Physical Requirements: Work performed
in a medical setting due to the need to work directly with patients
and/or residents. Must be able to sit, stand, bend, reach, talk,
hear, stoop, kneel, crouch, stretch, twist, use hands and fingers,
handle or operate objects, equipment, or controls and move about
facilities. Required vision includes close, vision, distance,
peripheral, and the ability to adjust focus. Capability to assist
residents with mobility tasks, including transferring them from
beds to wheelchairs, assisting with standing or walking, and
positioning them for therapy activities. May need to lift or
maneuver equipment such as therapy mats, adaptive devices or
weights. Maintain regular attendance and timeliness as set forth
within the work schedule designated by the department
supervisor/manager. The job requires accepting criticism and
dealing calmly and effectively with high-stress situations. The
ability to use your abdominal and lower back muscles to support
part of the body repeatedly or continuously over time without
"giving out" or fatiguing. Capacity to stand for extended periods
during therapy sessions and walk or move around the facility to
access residents’ rooms and therapy spaces. The ability to keep
your hand and arm steady while moving your arm or while holding
your arm and hand in one position. The ability to exert maximum
muscle force to lift, push, pull, or carry objects. Stamina to work
full shifts and perform repetitive tasks throughout the day,
including providing therapy to multiple residents and documenting
training sessions.
