About the RoleBaylor Scott & White Health is seeking a skilled Registered Nurse (RN) to join our dynamic Home Health team in College Station, Texas.Key Responsibilities

• Provide high-quality acute care services to patients enrolled in the Hospital at Home program.
• Conduct comprehensive assessments and develop individualized care plans tailored to each patient's needs.
• Collaborate closely with interdisciplinary teams to coordinate care and optimize patient outcomes.
• Act as a key educator and provide ongoing support to patients and their families, ensuring they have the necessary resources and understanding to manage their care at home.Requirements
  • Grad of an Accredited Program
  • Major - Nursing
  • Certification/License/Registration - Registered Nurse (RN)
